Artists Application Form

  1. IMPORTANT INFORMATION:

    In order to be considered for this juried event, please complete and submit the application and include a photo(s) of the art or craft you wish to display. Successful vendors will be notified of their selection, following which you will need to sign and submit a Waiver of Liability form and a temporary vendor license form and send them to the Town of Middleburg with the required $70 fee (includes the temporary business license fee) in order to complete and finalize your registration.

  4. Certification

    I am hereby applying as a vendor for the Artists in Middleburg or Oktoberfest event as indicated by the date below. I understand that this is a juried event and that submission of an application does not guarantee that I will be selected to participate. If selected, I understand that I will be required to sign and submit (1) the required Waiver of Liability form and (2) an application for a temporary business license from the Town of Middleburg, and to pay the required fees before my registration is complete and finalized.

  5. Electronic Signature Agreement*
    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
